- Current practicing license from the Nursing and Midwifery Council of Nigeria (NMCN).
- Clinical Experience:
- A minimum of one year of clinical experience post-RN qualification.

- Five (5) credits in O’Level examinations (WAEC, NECO, or NABTEB) in not more than two sittings, including English Language, Mathematics, Biology, Chemistry, and Physics.
- A recent passport photograph.
- Teller or receipt of payment.
- Scanned copies of O’level results.
