• Certificate in Community Health (Junior Community Health Extension Workers – JCHEW)
• Diploma in Community Health (Community Health Extension Workers – CHEW)
• Diploma in Medical Laboratory Technician (MLT)
• Diploma in Medical X-ray and Imaging Technology (XRT)
• Professional Diploma in Health Information Management (HIM)

Admission Requirements:

Prospective candidates must meet the following academic qualifications:

1. Certificate in Community Health (JCHEW): A minimum of three (3) O’Level credits in English Language, Mathematics, and Biology, with passes in Chemistry and Physics, obtained in not more than two sittings.
2. Diploma Programs (CHEW, MLT, XRT, HIM): A minimum of five (5) O’Level credits, including English Language, Mathematics, Biology, Chemistry, and Physics, obtained in not more than two sittings.
3. Direct Entry into CHEW (200 Level): Holders of JCHEW certificates who meet the above O’Level requirements are eligible for admission into the second year of the CHEW program.

   • Select ‘New Application,’ enter your name and a valid phone number, and click on ‘Create My Account.’
   • Log in using your phone number as both username and password.
2. Payment of Application Fee:
   • Upon successful login, confirm your information and proceed to payment.
   • The application fee is ₦6,500, payable via transfer, card, USSD, or Visa QR.
3. Form Submission:
   • After payment, complete and submit the application form online.
   • Print the application slip for your records.

Entrance Examination:
